• Kindly give consent for Vodacom to debit your bank account (do this by making sure to sign form on point 1)
• Vodacom has the authority to request documents if there is a need to do so in order to align with the business decision

Contract for a Company

• Valid Traders license
• Proof of income: (Consent letter bearing signature of person in authority and 3 months' bank statement)
• Consent to debit bank account by person in authority of the corporate (“do this by making sure to sign form on point 1”)
