We are offering a convenient funeral cover that you can be able to pay easily from your Vodacom Lesotho SIM Card. The funeral cover can be taken for only you or it can be for You and Your Family?

In the event of a death that occurs naturally we will pay out to you as the primary beneficiary, your chosen beneficiary or as per Lesotho Estate Laws. In the event of accidental cover, we pay double the cover, with no waiting period. Natural death waiting period is four (4) months.

You can pay premiums either on a weekly or monthly basis. You can choose different payment options for cover for you and cover for family. You have the option to review your options anytime. Bear in mind, new waiting period will be applied when changing amounts and not changing payment period.

Spouse is a person you are married to (whether by civil, customary, religious, or common law union). Only one spouse is allowed on one policy. You may change the spouse on the Insurance menu as and when required.

Member only
A maximum of 1 claim will be paid on Member only.

Family option
Children are own biological, legally adopted and/or stepchildren up to age 21.

A maximum of 7 claims will be paid on Family Cover Policy taken.

Who qualifies for this policy?
Anyone with a valid Lesotho National Identity and must be 18 years or older and 60 years or younger to apply for cover.

A spouse must also be 18 years or older and 60 years or younger to be covered under family at application of cover. Children should be under 21 years of age.

If you leave Vodacom network or migrate to Vodacom Contract tariff plan, You and Your family’s cover will come to an end.

Are there any added benefits to the cover option?
Yes. Each successful claim has a M300 airtime benefit.

Waiting period
A claim waiting period applies to everyone you have taken cover for, whether it be a spouse, children.

A waiting period of 4 months applies for natural death.

Accidental death has no waiting period.

A waiting period of 2 years for suicidal death.

New waiting period reasons:
1. When you increase your cover.
2. When you add/change spouse.
3. Cancellation of policy, lapsed policy.
No medical evidence is required to be eligible for the plan.

How do you get to register for this Policy?
To access the insurance, you either dial *111# and choose Insurance or dial *135# for a direct string.
Cover can be paid from airtime balance either on a weekly or monthly option. Please make sure to have enough airtime balance to cover your premiums to avoid cancellations.

How payments are collected to keep you covered:
1. Payments will always be collected on the same day of the week/month that the initial premium was paid unless there are insufficient funds.
2. A notification will be sent 48 hours before a recurring payment is deducted to serve as a reminder to the customer that the specified amount is due on the specified date.
3. In the event of non-payment, customers will be given a 15 (fifteen) days grace period.
4. Outstanding deduction will happen immediately when funds become available.
5. After 15 days of unsuccessful collection the policy will be cancelled, and the customer will be opted out.
6. Customers are allowed to pay the full outstanding amount during 15 days of the grace period.
7. Partial collections are not allowed. Only the full outstanding amount may be collected at a time.

When will the policy be terminated/cancelled/not paid out?
• The policy will be terminated upon death of the principal owner.
• No claim will be paid out if death occurs as natural death during the 4 months waiting period.
• No claim will be paid out in the deliberate involvement in an illegal act, riot, civil commotion, terrorist attack, or war.
• No claim will be paid out during the 2 years waiting period for suicide.

How will the insurance administrator know my beneficiary?
There will be an option to add spouse for family option or in the case of member only you can add a beneficiary of your choice. You should add a beneficiary that you would like to receive the claim in the event of death.
In case where no beneficiary is nominated, you will still have the option to provide beneficiary details at claims stage with our chosen insurance administrator Alliance Insurance, subject to proof of relationship to the deceased Member.
If there is any dispute as to who is entitled to receive a benefit in respect of this policy, the decision of the Insurer in consultation with Vodacom shall prevail.

Who will process your claims?
In the case of any death, including the principal payer’s death, the beneficiary will need to visit the nearest Vodacom store to lodge a claim.

When should you alert Vodacom Lesotho of the claim and when will the claim be paid?
Claim Notification period - claims will need to be submitted to Vodacom within 12 months of the deceased’s date of death for the claim to be valid.

Other terms to take into consideration for this policy:
1. Review of terms and conditions - The Insurer reserves the right to review all terms and conditions on this product. Premium rates will be reviewed annually and any resulting changes to premiums will be affected on 1 February of each year. Alliance will give one calendar month’s written notice of its intentions to make any alterations to the policy.
2. Fraud If any claim under this policy is deemed to be fraudulent in any manner through willful or fraudulent acts or, omissions and/or acts of dishonesty of the Main Member and/or the deceased Life Assured and/or the nominated beneficiary; the underlying benefits will be forfeited, the policy shall be cancelled, and no refunds of premiums will be made.
3. Cession Benefits under these policies may not be ceded, assigned, or pledged as security in any way.
4. Currency Benefits are expressed and payable in the legal tender of Lesotho.
5. Jurisdiction The laws of Lesotho, whose courts shall have jurisdiction in any dispute arising hereunder, will govern this policy.
6. Territorial limits: The plan provides cover for persons residing within the borders of Lesotho and who, is in a possession on National Identification. Cover is also provided to all Lesotho citizens irrespective of their country of residence.

The benefits of this policy are underwritten by Alliance Insurance Company Limited. Should you purchase this benefit, your personal details will be passed to the underwriter to underwrite, process and service this benefit. Please note that your acceptance of this benefit constitutes your consent in this regard.

DEFINITIONS
2.1 “Member/Client” is a Vodacom subscriber who has registered and has been making premium payments to the Vodacom Prepaid Funeral Insurance; and who is above the age of 18 years of age and not older than 60 years of age as at the Member’s Last Insurance Entry Date; and whose cover has been confirmed; and in whose name the policy is issued.
2.2 “USSD Platform” – The access where a Member/Client will access the Prepaid Funeral Insurance and register. They dial either *111# and choose Insurance or they go directly to *135#
2.3 “Life Assured” – The Member.
2.4 “Accident” - Any specific event, occurring at an identifiable time and place, that is unplanned, unexpected and violent, resulting from unforeseen and unintentional external circumstances and resulting in death.
2.5 “Beneficiary” - The Person or Individual nominated by the Member to receive benefits payable under this Policy in respect of a claim.
2.6 “Insurance” – Vodacom Prepaid Funeral Insurance.
2.7 “Vodacom” – Vodacom Lesotho, a limited liability public company duly incorporated in the Kingdom of Lesotho.
2.8 “Intermediary” – shall be a licensed insurance broker where applicable and to the extent that the “Intermediary” facilitates the provision of the insurance cover to the Insurance.
2.9 “Spouse” is a person married to the Member by civil law or tribal custom and shall include Common Law Spouse of the Main Member.
2.10 “Age” – The method for determining a Member’s Age shall be to calculate Actual Age, which is also referred to as Age Last Birthday.
2.11 “Commencement Date” the date on which the customer first made premium payments to this Policy of Insurance via Vodacom USSD platform.
2.12 “Member First Prepaid Insurance Entry Date” the effective date on which the Member first made premium payments to this Policy of Insurance and shall be recorded as the date of confirmation of cover to the Member.
